Care Coordination
Social workers provide long term care management assistance to at-risk
members age 65 and older. Assistance may include:
- Information and referrals to community services and resources such as home delivered meals, transportation, help at home, financial programs and services
- Counseling to address health and social changes affecting daily living
- Planning for future care needs
Emergency Department Follow-up
Phone calls to members 65 and older after an emergency room visit
Senior Care Calls
Friendly volunteer phone calls on a regular basis to isolated seniors

Special Needs Program
Staffed by Physicians, Licensed Clinical Social Workers and RN Case Managers. Provides annual evaluation, Care planning and Case management to specifically identified Kaiser Members who have both Medicare and MediCal.
