Care Choices: Inpatient Palliative Care TeamThe Care Choices: Inpatient Palliative Care Team is an inter-disciplinary team that assesses and helps to manage patient's pain and other symptoms. Outpatient Palliative Care ClinicAn outpatient clinic for members who have progressive life limiting medical conditions such as cancer, congestive heart failure (CHF), chronic obstructive pulmonary disease (COPD), chronic kidney disease (CKD) and have been referred to this clinic by their physician. A team of professionals including a physician, a nurse and a social worker meet with members and their families/caregivers in an initial 60-90 minute office visit. This visit may include a physician's examination, a consultation for education about the member's illness, a discussion of immediate and long-term concerns and the creation of a plan of care and follow-up.
